Hello,
Can you help me find a replacement? It's about 22 years old and I don't see any manufacturer or model number.

Sure, we can help. First, check to make sure that the installation hole spacing is 3-15/16", between centers. If so you can use either our 82-030A or 82-030B, depending on whether your key lock is centered or offset. I think it might be offset but it's hard to tell from these photos. Look over both products and see what you think.
