We should be able to help. The balance looks like our S385-34 channel balance, but that stamp of yours gives me pause. You may need to have this balance custom made with a special strength spring. Before we do that, can you confirm the weight of your sash? I'm thinking it might be 46 pounds, but I just want to be 100% sure. Thanks!
