Replacement part(s) for sliding door latching mechanism that broke off

Question

I need a replacement for the latch on our rear sliding door. I do not know who manufactured our door, but after about 12 years, the door latch actuator on the inside of the house broke off. The markings on the door glass are as follows:

CTG TEMPERED
16 CFR 1201 II
SGCC 1804 1/8 U

I have added photos of the latch (with the actuator missing)
