Replacement sliding door lock

Question

My sliding glass door is approx 16 years old. The smaller lock that moves left/right does not function properly and I'd like to replace the entire handle set.

I can't find a model number - only numbers are: "RD 1968 CAN" and "PAT3390557 US"

I tried using a general replacement from Lowes, but my door seems to be thicker, and the general replacement was deep enough into the door to allow the sliver "c" hooks to line up properly with the door frame.
