I have a 5/8" wide 17" long spiral window balance that I am trying to replace with the appropriate spiral balance and shoe pivot.

Thanks for reaching out to us. Take a look at the 5/8" Series 600 spiral balance with the blue tip option for tilt windows. The pivot shoe looks like it may be a match for the 15-188 or 15-178 depending on the thickness. You'll have to remove the old one and compare the specifications to confirm the best match.
