It's tough to make out your mortise from these photos, as I cannot see the whole thing, but the design is very similar to our 40-191. I would start by examining this part to see how well it compares to your original.

I have that EXACT same lock on my storm door. I took the lever to the hardware store as I thought I needed a set screw to tighten and I have lost the handle. Have you discovered the manufacturer?

I'm afraid we never received more information on this handle set. At this stage all I can tell you with confidence is that we don't sell a product with the same design. I'm very sorry for the inconvenience.
