I am wondering if it is possible to replace the lower hinge without taking out the side jamb? Is it possible to just remove the four screws holding the hinge and the sill and swing it out of the jamb? If not, there seems to be no way without completely removing the entire assembly to get a new hinge installed.

Thanks for posting! Yes, you will have to remove all the screws before you try to uninstall this pivot. If they are rusted in, then you will have to drill them out; I recommend that you use oil so as to put less wear on the drill bit.
You should remove the threshold, then swing out the bottom pivot out of the frame. You shouldn't have to remove the frame to do this.
