This appears to be an S770-34, with a stamp option of 34C. Look it over and see if you agree.

This is called a pivot shoe. It's tough to tell without seeing yours out of the window to know what you have. I think it might be an older version of our 15-188, but you'll need to remove it and compare its width and thickness to confirm. Let me know if anything differs. The video below shows how to remove and replace pivot shoes.
