Gorell Series 6000 (found on reciept).  There is a sticker on the door which has these codes NRFC: GOE-K-28-00004-00              /      PR2700             /

I cannot be certain what roller you need without seeing the original. Gorrell doors have used our 81-018 and 81-149 in the past, but you'll need to remove your old hardware and compare to confirm.
