I have identified the manufacture as Thermo Window Ind. in Altamonte Springs Fl. They may be in Longwood Fl now.
The window model info is DH-70 Model 700 High Sill

When I first saw your photos, I thought our 18-142, but that top sash guide comes with two screw holes. The closest one to yours is our black 18-009. Please review the overall dimensions to see what you think! I apologize that we cannot provide an exact match.
