Thanks for contacting us. I'm very familiar with this old top sash guide. This is the kind Milgard used to make, though they no longer do. I don't think you're going to find an exact match for this anywhere.

What we usually recommend for this is our 18-009. This is almost identical except for the fact that the top end has a lip that sticks out. You need to cut that lip off with a utility knife (or whatever tool you prefer) in order to get the clearance you need for a proper install. 

Look over our 18-009 in the meantime to verify that all other dimensions except that lip line up.
