We don't have an exact match for this, but see if our 18-174 can replace it. This has a similar design. It also looks like your top sash guide is installed backwards, in your pictures. It should be facing the other way. As long as the lip on the face of your old top sash guide matches up witht he lip on our 18-174, it should still work.
