Thanks for posting! This appears to be an older version of the 39-171 Truth Hardware Dyad Operator. The bracket the link arm is attached to can likely be replaced with the 39-546 Truth Snap-Stud Bracket. 

The compatible handles are the 39-114L or 39-205 (folding) crank handles.

Dimensions for 39-171

That is the casement window hinge. I believe you may need the 36-048 2-Bar Hinge Set. To confirm, you'll need to compare the dimensions. If the length of your track isn't 10", please let me know.

This set can be used to replace the top and bottom hinges, for both left and right handed applications. It is encouraged to replace both hinges at once to ensure they'll be a match.
