A landlord from Medina, OH says:
Number stamped on white part is 25 B and then Fiberlux. Also is there a diagram that shows how to reassemble. The parts were loose in window and I don't want to remove other windows for fear something will break. Thank you.

If you have a 19-002 guide on top of your balance and a 15-004 shoe on the bottom, you need Series 350. Let me know if those aren't the right attachments for you, and be sure to measure them when you check. It looks right, but the picture is hard to see.

If I had to guess, based on the number printed on your metal channel, I'd say you need a 2520 option for a 26" balance. How long is the channel? Select the Series 350 balance that matches the length of the channel by itself (no plastic attachments).

To check the option, remove your window sash (the frame/glass pane that slides) and weigh it. If my guess was correct, it will weigh between 9 and 15 pounds. If not, there is a chart on the webpage for your particular Series and length. It's in the product description, and it lists what option you need for a given range of sash weights.
