Looking for replacements. What causes the screw handle to come out of the socket when closing?

That worm gear comes out when the inside gearing of the operator is stripped. It can't be repaired, I'm afraid. You'll need an all new operator, but I can't tell what you have from this picture. It might be our 39-025. Start by carefully going over our measurements and pictures to see how well it matches yours.
