I believe these are called "balancer cams" or " top sash guides".
I have several broken in my home causing the block and tackle to not engage the sash.
The only markings on the windows AAMA certification stickers.

Thanks for posting. I am afraid we've never been able to find an exact match for this Top Sash Guide. Our closest is the 18-009. It lacks the side 'wings' your old guide has, but review the dimensions listed below and let me know if you think this could work as a substitute.
