I believe it is similar to the Truth Multi point vertical rod sliding door lock.

Here are photos of the handles and lock we need to replace. I belive it is called a vertical door lock for sliding door by Truth.

We did sell this handle set at one time, but it was discontinued and no longer made. Some customers have told me that our 82-030 may work as a substitute, but only if you are able to redrill the mounting holes in the door. Look it over and see what you think.
