Searching for milgard channel balance stamp 26 5 HR

Need a pair of replacement Milgard Channel Balances, stamped 26 5 HR Caldwell. End cap seems to be 1-1/16th inch (I think your 15-004 is supposed to be 1-3/32 but tough to measure to that)
I believe it is your 27" Series 385 Channel Balance

I agree, the S385-27 looks like our best match. The difference in shoe depth is likely due to wear. I'm sure yours was 1-3/32" at one time.
