Thanks for reaching out to us. Take a look at the S795-27 channel balance. Your old T-lock shoe appears to have an open cam so you will also need the 15-195B (sold separately) which is 1-1/4" wide. You'll just want to make sure the width matches up. Note that these shoes just pop on and off as shown below.

Your 26 5 stamp may translate to the 27E stamp but we recommend you weigh the window sash to confirm it falls within the 26 to 30 pound range.
