The piece looks to be about 1.25"wide x 1.5" tall x 3/4" deep.

This looks like the 15-192 Pivot Shoe to me. Note that the Simonton version of this shoe has a slightly different metal cam shape, so you'll want to swap your old one with the new one to ensure pivot bar compatibility.
