Slider handle replacement

Hi I need a new slider door handle. I have attached a picture of the tag and what the handle looks like. I am having trouble finding this so any help would be greatly appreciated.

Thanks for writing in! Typically, we see damage like this when excessive force has been applied to a door handle. This usually indicates that the rollers on the door may be sticking due to debris in the track, damage to the track, or (most often) because they need to be replaced. This is also something you may want to consider in your repairs.
