Sliding Door Handle

Please help us in finding a replacement part for this handle. It was new in 2005 on a builder home, so we have no idea where it came from. No i.d tags or markings on door itself? it measures 6 1/2 inches in overall length and screw holes are 5". Any help will be appreciated.
