Sliding door handle

A handy person from Irvine, CA says:
Merry Christmas! I need the entire assembly. I am OK with a new key. Outdoor side has key. Indoor side has latch. Not a mortise lock; nothing slides into the door frame. The lock works by just grabbing a groove on the side of the frame. No marking found on parts. Holes for bolts are 4 7/8" apart. The center hole is 2 1/2" below the top screw hole.
