Need to replace sliding glass patio door hardware. The holes in the door are 5 3/8th apart and it is also a surface mount.

Hello,

Take a look at the 82-009A handle set. We call this the gripper lock which yours seems to be. It has that type of clamp that is activated when the lever is moved. I believe this is the handle set you need to use but you will need to drill new mounting holes. Compare your lock with the 82-009A to confirm that this could work for you. Thanks for posting!
