The 15-048 comes to mind. You'll want to compare our specifications with yours to confirm. We've seen this pivot shoe used with the Series 600 spiral balance but that doesn't appear to be what you have. Can you remove an undamaged balance assembly from the window and show us photos? Can you also tell us or show us a photo of how the balance installs at the top of the window?
