Hello! The Unique balance manufacturer has been out of business for some time now. We believe you have the S390-22" side load balance which is attached with the 19-002 top nylon fitting and 15-001 1-1/8" deep bottom nylon shoe. We recommend you measure as shown below to confirm. You can also select the 2120 stamp option for your replacements.
