Your operator resembles the 39-253RB and 39-253LB that uses the 39-529 track slider at the end of the straight arm. Compare the overall dimensions to confirm a good fit! 

Also see the 39-452 and 39-451 or the 39-376 and 39-375 cover/handle depending on style preference. 

For a new hinge assembly, please start by reviewing the 36-048 that has a 10" sill track or the 36-049 with a 14" sill track. If your track is longer, please let me know! Note that both the top and bottom hinges will need to be replaced at the same time.
