It looks like you have Truth hardware, which would generally use a handle like the 39-114L or folding 39-205 that attaches to an 11/32" diameter spline. If you need the entire operator assembly, we should be able to supply a replacement but we would need you to open the window and show us the arms that move the sash to compare to our inventory. Thank you! 

The operator shown in your photos resembles the Entrygard 39-031 at first glance. We also offer the 39-032 opposite-handed version. The determining factor would be how the length of the shorter link arm compares to yours. Review the diagram below to confirm it's a match
