Siteline casement circa 2006 top sash hinge. The small piece that is loose was attached making it 8 3/4" arm looking for a left and right.

We don't sell the exact hinge you show here, but the good news is that you should be able to use our 36-048 as an alternative. The catch is that you have to replace the top and bottom at the same time.
