Thanks for writing in! There is a good chance we can provide you with a replacement recommendation for your coil balance assembly, but I'll need to see a clearer photo of the shoe to identify it. Unfortunately, the photo is a bit blurry so it's a bit difficult to make out exactly what you have.
Would you be able to try resending the photo? Please also confirm if there are any stamps/markings on the metal coil. If there aren't, I'll need to know the exact weight of your sash.

Thanks for the photos. We do not have an exact replacement match for your coil shoe but our 15-191 Heavy Duty Pivot Shoe should work as an alternative. Be sure to look over the dimensions to confirm that this will be a fit.
The compatible coil balance is the 1/2" thick Series 160 Coil Balance. Since your sash weighs 12lbs, I suggest using the S160-06 Coil Balance. This spring uses the 15-150S Coil Single Support.
