Thanks for reaching out! Your pivot shoe appears to be an older, discontinued Thermal Gard shoe. I believe our 15-070 should work as a suitable replacement for your existing shoe. 

This shoe is compatible with the 26-356 Pivot Bar Set, also a Thermal Gard brand part. Unfortunately, the nylon housing is no longer available, so you'll need to reuse your existing housing, providing they are still in good, working condition.
