Top Sash Guide & Clips

I recently purchased two new Series 351 1/2" channel guides, but when I tried to install them I discovered that the top caps (sash guides) were broken as well.

Attached are photos of the existing broken guides. I need a right and left guide for one window. Please tell me the specific part number I need.
