Top Sash Guide for PGT Single Hung Window

Looking for a replacement top sash guide for my PGT window. Photo of good part attached. There are no markings on the part. I am unable to find this part on your web site.
Can you assist in identifying this part?

Thanks for the quick reply. A couple of comments and questions.
1. My length is 1-1/2 inches, your 18-015 part length is not given?
2. My width is 3/4 inches, your 18-015 part width is given as 13/16?
3. My part has 2 screw holes for mounting, your 18-015 part only has 1 screw hole for mounting?
4. My top mounting hole center line is approximately 3/16 inches, your 18-015 part mounting hole is marked as 3/8 inches?
Would you kindly clarify why you think your 18-015 matches my part.

Here are our dimensions compared to yours:

1. 1-1/2" height
2. 13/16" width
3. Only the top hole is used for mounting.
4. Yes, it is 3/16" down to the center hole
