Missing just one of these guys.  There should be one on each side on the window frame to engage the ballast.

It's a little difficult to tell what replacement top sash guide will work without knowing the measurements of the part. You will want to begin with the 18-009 guide. Review the measurements carefully to see if this will make a good match for you.
