Thanks for reaching out! We do not carry an exact replacement for your existing top sash guide but there is a chance that either our 18-008 or our 18-180 could potentially work as a replacement. The the 18-008 does not have the extended plastic tabs on the top of it but that should not effect the function of the window.

You should be aware, when installing them to the sash, you will need to modify either of these guides to accommodate the installation hole spacing. It is important to note that if any modifications are made to the parts, they would not be returnable.
