Hello Bob,
I saw this sash and believed it was the closest one to what I currently have. However, it doesn't look identical.

The 18-178 pair is designed as left and right-handed guides. Your old top sash guide looks like it was designed to be neutral; but so long as the dimensions match I don't think this will be an issue. Is there any reason you think it wouldn't work?
