Thanks for posting. For your top sash guide, see our 18-166 or 18-009. Compare both with an undamaged version of your top sash guide to see what matches best. For your channel balance, confirm that the length of the metal channel, not including the nylon attachments, is 30" long. If so, see either our S380-30 or S385-30. The only difference between these two balances is the 'depth' of the bottom nylon shoe, so compare both with your old hardware to determine the best match. You will need a stamp option of 2940.
