Start by looking at our 39-031 and 39-032 Truth EntryGard casement window operators. I think you'll find these match up quite well to your existing hardware.

Is it possible to replace the spline (I think that's the right name) the cylindrical piece that connects crank to the single arm mechanism. This appears to be the worn part and nobody makes an 11" surface mount

It's not possible to replace the spline, no. Once that strips out, the operator is shot and the entire mechanism will need to be replaced.
