The roller is worn and coming off the track.  Not sure what to buy to fix/replace.

Hey there!

Thanks for waiting during this busy season. This operator looks like an older version of the 39-028E. The arm is 9-1/2" long when measured from the center of the gear to the center of the roller wheel. You'll want to measure as shown below to make sure yours does as well.
