This is from a single rail mounted in the center of the drawer.

There is a track in the cabinet which the track under the drawer glides along.

The plastic "sledge" in the picture is like a short "T". The rail in the cabinet is wider towards the front (where you would stand) and the wider portion of the "T" slips into, as you push the drawer in, the wider portion of the "T" prevents the drawer from tilting up and keeps it in the groove.

I recently purchased this home and just noticed that two of the several cabinets need new 'sledges' (I'm not sure if that is the correct term; however, the closest part I found seemed to carry that name.
