1) I have identified the channel balance that I need S385-35". I need a little help with the sash weight. The attach picture includes the stamp on the channel. would this mean 34 pounds?
2) what is the part number for the small plastic piece attached to the window frame in this picture?

Hello there. For a channel balance stamped 34-3, choose stamp option 3430. The nylon block is called a Top Sash Guide. Yours might be the 18-009 or 18-166, but compare with an undamaged one to see if either of these options match what you have.
