Thanks for reaching out to us. We don't sell this exact hinge, regrettably. However, take a look at the 36-048. Carefully review the specifications and see how they compare with your old assembly. We just recommend you replace the top and bottom hinges at the same time.

Unfortunately, my part is not the same dimensionally to the part you suggested.
Per figure 2 (Additional Dimensions), the horizontal arm is 8 3/4" overall and hole spacing is 8 1/16"
Also, the horizontal arm is mounted 1/2" above the plastic guide.
I've attached a photo. I help you have another option. Thank you
