Ultra weld crank replacement

I'm sorry I don't have a photo of this on the window. I think this is from a Polaris Ultraweld window. We finally found one but it came without the set screw. Could you possibly identify it and tell me if we might be able to purchase from you?

Thanks for reaching out to us. Before I recommend anything, can you show us a photo of the cover this handle goes with? It would also be helpful if we could see a photo of the base of the operator with the cover removed. That should help narrow down some options.
