I think this part is perhaps a Top Sash Guide. There is no part number on it.

We offer a top sash guide similar to yours, however it includes two holes instead of the one oblong hole you are showing. Please visit our 18-142 guide. Even with the minor difference, we believe it should still fit your window sash. Take a look. What do you think?
