Upper Window Balance Guide?
The part you are replacing is the top sash guide. These do help to guide the balance and the window sash (the pane) when opening and closing the window. They also help to prevent the balances from shooting out of the window and keep it centered.
I'll need to see a front facing view of the original to identify its replacement.

Thanks for the additional photos. Your window balance appears to be assembled with a wingless top guide and a winged 7/8" shoe. We can supply the replacement but it will need to be a custom assembly, since we'll need to add the 15-029 shoe to the balance.
I'll just need to know the number of balances you'd like to purchase to get the quote made up for you. The quantity minimum is (2) balances.
Regarding the to sash guide. Unfortunately, we do not have an exact replacement match for the one you have. There is a possibility the 18-057 Top Sash Guide could work for the repair but we cannot make any guarantees. Be sure to review and compare the dimensions below.
