Thank you! I think our best option for you is going to be our 80-106. This is a fairly versatile patio glass door track that works very well in doors like yours, at least in my experience. It may seem like it's too small at first, but the benefit of that is that you can adjust it to wherever it needs to go in the space available to accommodate a wider range of doors. The video below will go into more detail as to how you can use this in your application.
I think I can help you with that question. Yes, you can install the 80-106 on to concrete. However I would install some type of barrier between the 80-106 and the concrete.
Also, looking at your second photo, you might need to build up the track a little. See if you can find some type of thin material to install under the 80-106 to keep the track the same height. I like to use wood but it's really up to you.
