Thanks for reaching out to us. I'm not sure if we'll be able to help with this repair. To help us confirm, can you show us a photo of the operator the cover goes with? We'd like to see the base and the arms, specifically. 

We can't identify the lever lock when it's installed, however. Seeing it removed from the window is the only way we can help.
