Weather Stripping Replacement for Exterior Door

Good morning.

I'm looking for a replacement item for the deteriorated weather stripping on my son's exterior door. I've attached 2 profile pictures of the weather stripping along with an approximate sketch for dimensional purposes.

Hopefully you'll be able to fine a replacement part. I do not know the door or frame manufacturer. Approximately 15 years old?
