Weather strip on a wood patio door.

I need the weather strip for my wood sliding door. Home built in 1979 and this is the original door. All of my windows and doors are heavy wood, and the only hardware is on some of the sliding windows Caradco gold hardware. Weather strip is in vinyl and is cracked, falling apart. Height is approx. 78". Thank you for any help you can offer. Nancy
