If possible, please ensure that the sample you show us is as undamaged as possible. I understand that your original is probably brittle, so this is easier said than done, but please try your best. Figure that the more of it is deteriorated or torn off, the less likely it is that we can help.

Please also note that we cannot garner precise dimensions from a photo of a tape measure. All relevant sizes must be written out as precisely as you can manage.
